Full Circle is an in-network provider for the following insurances

  • Blue Cross Blue Shield
  • United Health
  • WPS
  • Humana
  • Empathia/Life Matters EAP
  • Please note that Full Circle is not a provider for BadgerCare, Medicare or Medicaid.

We accept all other insurances on an out-of-network basis. Some insurance companies do provide out-of-network coverage and we would be happy to discuss with you what your most cost effective option would be to cover the costs of counseling.

The best progress is made by those clients who commit to meeting weekly for 50 minute sessions, but ultimately the frequency with which you attend sessions is up to you.  How often you attend sessions, your readiness to make changes, and your unique needs and circumstances will all affect how long your process will take.

Location Hours are as follows:

Appleton:

  • Monday:        9:00 a.m. – 1:30 p.m.
  • Tuesday:        9:00 a.m. – 6:00 p.m.
  • Wednesday:   9:00 a.m.- 1:30 p.m. and 5:30 and 6:30 p.m.
  • Friday:            9:00 a.m. – 3:00 p.m.

New London:

  • Tuesday:         1:00 p.m. – 6:00 p.m.
  • Friday:            9:00 a.m. – 1:30 p.m.

 

Counseling Sessions are available by appointment with specific counselors as follows:

Julie: 

  • Monday, Tuesday, Wednesday:  9:00 a.m. –  1:30 p.m. in  Appleton
  • Friday:  9:00 a.m. – 1:30 p.m. in New London

Connie: 

  • Tuesday: 1:00 p.m. – 6:00 p.m. in New London
  • Wednesday: 5:30 p.m. and 6:30 p.m. in Appleton
  •  Friday: 9:00 a.m. – 3:00 p.m. in Appleton

 

 

 

When Using Insurance

I have standard set fees for my counseling services, but the amount you pay will depend upon your specific insurance coverage. As a service to you, I provide an instruction sheet prior to our initial session that details the steps of learning all about your mental health coverage. If any questions remain after completing this form, I am happy to talk with you to ensure you fully understand your coverage and your portion of the financial obligation.

All co-pays, co-insurance and/or deductible amounts are due prior to the start of each session and payable in cash or check only (no credit cards are accepted for payment when using insurance).

 

Private Pay Option

Many clients prefer not to use their insurance; I encourage you to read more about the risks and benefits of using insurance so you are fully informed before making your decision. Private pay fees with Julie are $150 for the initial session, and $135 per individual  or couple/family session.

We also offer a reduced private pay option with Connie, an LPC-IT (Licensed Professional Counselor In-Training) who is completing her necessary 3000 hours of supervised counseling experience in order to become a Licensed Professional Counselor.  Connie’s private pay fees are $85 for the initial session and $75 per individual or couple/family session.

Private pay fees are payable in cash, check, or credit card and all fees are due prior to the start of each session. Health Savings Account/Flex Spend cards are also accepted.

I have two office locations for your convenience;  477 S. Nicolet Rd, #12, Appleton WI 54914 and E8172 Cut-off Road, New London, WI 54961, which is located 15 miles south of New London, and approximately 30 minutes from Appleton, Oshkosh, and Waupaca.
